Every provider in the network completes a one-time, NCQA-standard credentialing process before seeing a single patient — the same credentialing standard used by the leading healthcare institutions in the country.
When you submit a CuraVita360 clinical intake, it is routed to a provider whose specialty, licensure, and clinical expertise match your program and your state. That provider reviews your full health history, assesses clinical eligibility, and makes an independent prescribing decision — not a rubber stamp. If the program is not right for you, they will tell you, and you will not be charged.

You complete a detailed health questionnaire covering your medical history, current medications, goals, allergies, and any contraindications. This takes 5–10 minutes and forms the foundation of your provider's clinical review.
02 Identity Verified
Persona.com verifies your identity via government-issued ID and a live selfie match. This step protects you and ensures our providers are treating who they believe they are treating — a critical safeguard in telehealth.
03 Provider Reviews
A licensed, board-certified provider whose specialty matches your program reviews your complete intake within 24 hours. They assess your health history, evaluate eligibility, and determine the appropriate treatment plan for your specific clinical picture — independently and without pressure to approve.
04 Provider Decision
If approved: your provider creates a personalised treatment plan and transmits an electronic prescription via DoseSpot Surescripts. If not approved: you are not charged — a full refund is issued automatically, no questions asked.
05 Rx + Blockchain Record
Your prescription is transmitted to our pharmacy partner (Olympia 503B or Empower 503A) and a cryptographic hash of your prescription record is written to a permissioned blockchain. You receive a verifiable credential in your CuraVita360 app wallet — your prescription record, permanently yours.
06 Delivered + Ongoing
Your medication arrives discreetly within the estimated delivery window. Monthly check-ins with your provider keep your treatment on track. Lab orders, dose adjustments, side effect reporting, and clinical questions are all managed through your secure CuraVita360 portal.

State-matched licensure
Providers prescribe only in states where they hold an active, verified license. Our platform automatically routes intakes to eligible providers — you will never receive a prescription from a clinician not licensed in your state.

Full refund if not approved
