CuraVita Health Technologies, Inc. ("CuraVita," "we," "us," or "our") is committed to protecting your privacy and the confidentiality of your personal information, including any health information you share with us.
Information We Collect
We collect information you provide directly:
- Account registration information (name, email, password)
- Shipping and billing addresses
- Purchase history and subscription information
- Wellness quiz responses and habit log data (non-PHI)
- Communications with our support team
For clinical services, protected health information (PHI) is collected separately and governed by our HIPAA Notice of Privacy Practices.
HIPAA Notice of Privacy Practices
For patients using CuraVita Clinical services, your Protected Health Information (PHI) is handled in accordance with HIPAA (45 CFR Parts 160 and 164). We maintain a Business Associate Agreement with all vendors who may access PHI. You have the right to access, amend, and receive an accounting of disclosures of your PHI.
How We Use Your Information
- To process and fulfill supplement orders and subscriptions
- To facilitate clinical program enrollment and provider review (PHI governed by HIPAA BAA)
- To send transactional communications (order confirmations, shipping updates)
- To send wellness marketing emails (only with your consent; never PHI)
- To improve our platform and personalize your experience
Data Retention and Deletion
You may request deletion of your account and personal data at any time by contacting us at privacy@curavita.com. We will complete deletion within 30 days (GDPR/CCPA compliance). Clinical PHI is retained as required by applicable state and federal law.
